LOCAL SERVICE
MEDICAL EVACUATION INDONESIA
All services described below are provided by XIVA when Participants travel within the territory of Indonesia. The services that can be elaborated one by one are as follows :

Emergency Medical Transportation with Medical Companion
The service is provided by XIVA at no additional cost to Participants, Company/Participants' Health Insurance if they receive medical recommendations and referrals from XIVA.

Repatriation of Participants after Inpatient Care with Medical Companion
The service is provided by XIVA at no additional cost to Participants, Company/Participants' Health Insurance if they receive recommendations for post-inpatient care transportation and medical recommendations from XIVA.

Membership Case Monitoring
No fees are charged to Participants unless XIVA uses a third party for Participant monitoring processes. Costs will be confirmed to Participants, Company/Participants' Health Insurance in advance.

Post-Inpatient Care Benefits
Paid up to IDR 14,000,000,- (fourteen million Indonesian Rupiah) and no more than 30 (thirty) days and points (1) and/or (2) occur, only if they receive recovery recommendations from the attending physician and are known to XIVA, by submitting compensation for incurred expenses. Benefits cannot be carried out at the Participant's home or family home. Must be done at a facility and not a medical facility.

Emergency Medicine and Prescription
Medication is the responsibility of the Participant. Transportation costs are borne by XIVA.

Emergency Cashless Facility
All inpatient costs are the responsibility of the Participant, Company/Participants' Health Insurance, including third-party administrative costs, with assistance from a third party to provide cost coverage on behalf of XIVA. Billing expiration is not applied, and the Company/Health Insurance pays all bills in full amount including bank transfer fees which are borne by the Company/Participants' Health Insurance. A deposit of funds to XIVA will be applied for this service as per agreement.

24-Hour Doctor Assist
The service can be accessed and provided 24 hours a day by XIVA in Indonesia.

Repatriation of Remains.
The service is initiated, decided, and paid for by XIVA to send the remains to the final destination of the funeral home using air cargo.

Doctor or Specialist Dispatch.
The cost of sending a doctor or specialist is borne by XIVA for the purpose of evaluating whether services (1) and/or (2) are necessary. The service is paid for, implemented, and decided by XIVA.

Traveling with Minors.
An additional companion will be provided by the airline for minors. Commercial flight tickets will be provided by XIVA at no additional cost to Participants, Company/Participants' Health Insurance.

Traveling with Companion
No additional charges are incurred to Participants, Company/Participants' Health Insurance for this service.

Repatriation After Inpatient Care
One ticket will be provided to Participants by XIVA post inpatient care if services (1) or (2) have been provided at no additional cost to Participants, Company/Participants' Health Insurance.
